A Different Kind of Start San Diego’s coastline faded quickly behind you, replaced by a landscape that felt stripped down to its essentials — dust, heat, and distance. The desert didn’t hide anything. It didn’t need to.

Sofia Ríos sat behind you, already organizing paperwork that seemed to multiply the more she touched it. “We’ll be moving between fixed clinics and mobile units,” she said, not looking up. “Schedules will change. Priorities will change faster.”

Dr. Mateo Álvarez leaned forward slightly, resting his arms on the seat in front of him. Unlike Elina, he spoke more — but carefully.

“These areas don’t get consistent support,” he said. “When something goes wrong, it stays wrong longer.”

Sofia finally looked up.

“And when we show up,” she added, “we don’t get to be wrong at all.”
